Life Care Planning

A tool used in litigation to estimate the medical and non-medical needs of persons with catastrophic injury or chronic illness over a lifetime. A dynamic document based on medical record review, comprehensive assessment, and collaboration with providers. A life care plan is an individualized plan of care that includes primary and specialty physician evaluations, nursing care, therapies, medications, testing, home modifications, medical equipment, transportation and anything else related to the injury or illness that incurs cost over a lifetime.
